[A] Budget: Looking for Proposals
[B] Scope of Service:
(1) Vendor needs to provide self-isolation support for program administration and case management to the government authority located in Fairfield, CA.
- Administer the self-isolation support program (program and fiscal monitoring, management, and reporting).
- Receive referrals for the Program from community partners (hospitals, shelters, congregate living facilities, etc.), screen referrals to ensure program eligibility criteria is met, coordinate services (lodging, meals, room cleaning), and conduct intake with each participant.
- Provide ongoing case management/case navigation for each participant throughout their stay in the program to connect them to community resources and support, and ensure they are complying with program rules.
- Case management and case navigation includes the following processes: intake, needs assessment, service coordination, monitoring and follow-up.
(2) All questions must be submitted no later than September 16, 2022.
(3) An optional intent to propose will be due on September 14, 2022.
(4) Contract term will be one year.
