(1) Vendor needs to provide enhanced geriatric case management services to the government authority located in California. – Contractors will be expected to assist Veterans with housing maintenance, geriatric support services and case management services.– This contract requires that Contractor maintain a minimum staffing ratio of one dedicated, full time, on–site primary daytime medical and nursing case manager per a maximum of 12 Veterans.– There will also be secondary medical case management staff coverage for all remaining day, night, and weekend hours.– The primary medical case manager will provide structured individual case management, including counseling on self–care skills, adaptive coping skills, financial planning, permanent housing maintenance, written care plan, referral for financial benefits, socialization, managing medical issues and all other needs as applicable.– The primary medical case manager will be required to have daily check ins during their normal tour of duty times.– The secondary daytime and night and weekend medical case managers will ensure Veteran basic needs are being met and continuity of care through daily check ins, medication oversight and documented case management follow up as outlined by the primary case manager.
