(1) Vendor needs to provide case management service to the government authority located in Pennsylvania. – Partners providing Office of Children and Families (OCF) funded services in community schools i.e. out– of–school time, career connected learning, and case management services.– Dedicated City employee assigned to each Community School to support strategic partnerships and programs that promote wellness, stability, and learning opportunities for students, families, and neighbours.– Provider offering expanded learning support opportunities outside of the school day, including after–school, summer, and youth workforce.– Platforms used to engage and connect with families, share resources, and solicit their feedback.– Evidence–based parenting curriculum or workshop to support educating families on community resources or school–based resources.– Case Managers are degreed professionals that are preferably community members that support families navigating social supports, socio–economic programs, health supports, and school supports.– A professional hired by the organization responding to this Request for Proposals who supervises the case managers and supports school–level work and key deliverables as decided by the provider.– Case managers will support families in navigation. If there are short term costs that are needed to access resources, the case manager will be able to use funds from their agency’s discretionary emergency funds.– Providers will also use funding to provide attendance incentives for individual students. The funds can be used to support individual, class, or school–wide celebrations supporting increased school attendance.(2) A pre–proposal meeting will be held on May 17, 2024.(3) All questions must be submitted no later than May 24, 2024. (4) The contract term will be five years.
