(1) Vendor needs to provide case management service utilizing opioid settlement funds to the government authority located in New Jersey.– To provide case management services and support to residents treatment for opioid use disorder. • Data collection– Total number of case management service hours provided.– Total number of persons served– Confirmation of completing 7 core functions of case management – o Screening and case intake.o Assessment and evaluation.o Risk evaluation.o Plan and establish goals.o Implementation.o Monitoring and review.o Outcome evaluation.– Budget detail/Audit trail(2) The contract period will be one year.
