(1) Vendor needs to provide incident management tracking system.– Transition phase– A transition phase, in conjunction with the to establish operations and protocols related to the administration of the CCMS program.– Transition Period at start and end of contract– Overall, Contractor Tasks and Activities– he Active Case Management Phase are articulated within the following Sub–phases:– Case Management and Community Supervision;– The Case Management and Community Supervision Sub–Phase, the Contractor must provide support services to individuals enrolled in the CCMS program, which may include but are not limited to:a. Compliance monitoring;b. Linkages to health support;c. Linkages to mental health assistance;d. Linkages to addiction and substance abuse counselling and support;e. Information related to housing and employment;f. Information related to child–related or family needs; and,g. Information related to 2SLGBTQI+ community.(2) The contract period will be for three years.
