(1) Vendor needs to provide intensive case management service to the government authority located in Bakersfield, CA.– Comprehensive Assessment providing a detailed understanding of each client's needs, including mental health, substance use, physical health, and housing. Assessment should be ongoing to adjust services as circumstances change.– Individual Care Plans tailored to each client, addressing their specific barriers to stability, such as untreated mental health disorders, addition or chronic illnesses. Plans should establish realistic, achievable goals.– Coordination of Services with a range of services providers, including housing agencies, healthcare providers, mental health services, substance use treatment, employment services, legal aid, etc.– Trauma–Informed Care, recognizing that chronically homeless individuals have experienced significant trauma, services should be delivered in a way that minimizes re–traumatization and promotes safety and healing.– Client Advocacy, helping clients navigate complex systems like public benefits, housing applications and ensuring they receive the services they are entitled to.(2) A non–mandatory pre–bid meeting will be held on October 16, 2024.
