(1) Vendor needs to provide supervised release case management services to the government authority located in Wisconsin.– Improve quality of life for individuals and communities by promoting hope, health, and healing– Arrange for the control, care, and treatment of individuals court ordered for supervised release in the least restrictive manner and in accordance with the plan for supervised release approved by the court.– Support the provision of case management services for clients on supervised release– Pre–release planning, assessment, and communication– Home visits, telephone contacts, and collateral contacts– Billing for services and program related expenses– Assisting clients with problem solving– Money management skill development– Oversight of client activities and requests– Any provision deemed to be appropriate and/or court ordered to ensure the safety of the community– Provision of psychosocial, suicide risk, mental health, and other risk assessments– Preparation of client calendars– Agency fulfils this requirement in part through community services, including case management and associated services, provided to clients through a contracted service provider.(2) All the questions must be submitted no later than October 18, 2024
