(1) Vendor needs to provide case management services to the government authority located in New Jersey.– To develop a case management plan based on the individual’s expressed interest and need.– Case managers will help connect individuals with services/ resources in their area and entitlements, and assist them in securing permanent housing as quickly as possible. Individuals served in this program will have access to agency funded housing subsidies.– It is anticipated that this funding will lead to the development of case management services to ensure adequate staffing, facility costs, outreach activities, and participant assistance. (2) All questions must be submitted no later than October 28, 2024.
