(1) Vendor needs to provide case management and recovery support services to the government authority located in New York.– Conduct comprehensive assessments and re–assessments as needed utilizing a validated riskand needs assessment tool, screen to identify risks and needs for all accepted participants.– Provide comprehensive case management, individualized service coordination, andsubstance use disorder recovery services– Develop individualized, culturally and linguistically appropriate treatment plans and workclosely with treatment providers– Support client recovery by providing dedicated staff for services such as vocational andeducational assessments, job placements, and peer specialists– Promote ongoing self–care techniques and wellness and recovery activities for families andtheir loved ones– Facilitate virtual monthly family caregiver support groups.– Provide appropriate supervision and oversight for any staff included in this project(2) All questions must be submitted no later than November 15, 2024.
