(1) Vendor needs to provide non–clinical case management services to the government authority located in Cheyenne, WY. – To effectively prepare, track, and assess the medical readiness of agency service members according to the service member’s specific medical readiness requirements to achieve the highest levels of medical readiness and deploy ability for assigned federal and state missions.– A coordinated interdisciplinary plan of care for a Soldier from injury/illness/disease through returning the Soldier to duty or medically separating the Soldier from the organization.(2) The contract period will be one year.(3) This bid is Set–Aside for Total Small Business.
