(1) Vendor needs to provide information and referral case aide services to the government authority located in Lancaster, PA.– By enabling the elderly person to live with as much independence as his or her condition permits.– By identifying elderly persons who are in need of services, particularly those who have a low income, are sixty years of age and above, isolated, living alone, minority or frail.– By providing information to elderly persons regarding aging and community services as well as how to access them.– By assisting elderly persons in obtaining needed services, including transportation escort service (when necessary), and by providing continuous service monitoring to ensure consumer satisfaction and service appropriateness.– By advocating on behalf of the elderly person.(2) A contract period will be for three years.
