(1) Vendor needs to provide case management services to the government authority located in Stillwater, MN.– To provide case management services with a strong focus on person–centered care and individualized support, as well as organizations that have a focus on providing mental health services.– Development of the Person–Centered Support Plan– Informing of Service Options– Services that support vulnerable adults, children and individuals with developmental disabilities– Consulting with Relevant Medical Experts or Service– Assisting in Identifying Potential– Assisting in Accessing Services and Appeals– Coordinating Services– Evaluation and Monitoring of Services(2) All questions must be submitted no later than February 7, 2025.
