Government Authority Located in Riverside, California; USA based organization looking for expert vendor for housing case management services.
[A] Budget: Looking for Proposal
[B] Scope of Service:
(1) Vendor needs to provide housing case management services to the government authority located in Riverside, CA.
Case Management
• Build initial trust and rapport through strong engagement and relationship building techniques with Clients.
• Develop, implement, and revise individual case plans and consistently monitor Clients’ progress toward their goals using a Housing First approach and in alignment with the CES process
• Provide on-going person-cantered supportive and/or case management functions in accordance with Client case/treatment plans, which could include home visits, apartment checks, supervision, and crisis management and linkages to interventions, as needed and appropriate
• Conduct assessments of household needs, strengths, and challenges
• Implement core principles and evidence-based practices, including Housing First, Motivational Interviewing, and Trauma-Informed Care
• Provide linkages and referrals to community-based resources and programs
• Assist with helping the household increase income either through employment, benefits, or both.
Collaboration and Communication
• Serve as a professional liaison for the City of Riverside Office of Homeless Solutions, conveying needs, requirements, and expectations related to Housing Case Management services
• Participate in the Coordinated Entry System as appropriate
• Attend weekly case consultation meetings and monthly trainings
• Meet regularly with the Housing Authority Manager to exchange pertinent information, providing ongoing updates and keeping them informed of issues and concerns related to clients
• Communicate appropriately as needed with probation officers, social workers and other care providers, and family members
• Maintain a high level of ethical conduct regarding privacy and confidentiality, dual-relationships, and professional stature
(2) All questions must be submitted no later than August 08, 2019.
(3) Contract period will be for one year.

- Onshore (USA Only);
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
