[A] Budget: Looking for Proposal
[B] Scope of Service:
(1) Vendor needs to provide behavioural health housing support services to the government authority located in Santa Rosa, CA.
- To provide intensive case management services for homeless individuals from the Joe Rodota Trail who have been placed in permanent supportive housing,
- To identify a community-based organization to provide intensive case management services to occupants of the JRT that have been transitioned into permanent supportive housing arrangement,
- Operate intensive case management programs designed to provide an array of intensive community-based services designed to maintain clients as successfully housed,
- Maximizing the number of individuals that are housed within these residences,
- Providing behavioral health and other supportive services necessary to ensure successful transition to housing in the community.
(2) All the questions must be submitted no later than January 8, 2020.

- Onshore (USA Only);
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
