Government Authority located in Clovis, California; USA based organization looking for expert vendor for adult re-entry planning program case management services.
[A] Budget: Looking for Proposal
[B] Scope of Service:
(1) Vendor needs to provide adult re-entry planning program case management services, Evidence-Based Practices case management services using Risk, Need, Responsivity principles offer proven ways to improve outcomes for the target population, the greater the use of evidence-based practices in probation supervision and services, the greater the recidivism reduction to the government authority located in Clovis, CA.
- Case managers that will conduct an evidence-based needs assessment and develop a case plan for each participant in the program,
- The assessments and case plan should be completed a minimum of sixty days prior to participant being released from jail,
- The assessment and case plan will result in the identification of such critical factors as job training/employment, education, substance abuse treatment, life/coping skills, mental health services, antisocial cognitions, family/marital relations, leisure/recreational activities, housing, and food security, among others,
- Once the stabilization and criminogenic needs have been identified and the case plan has been completed, the case manager will address those needs through referrals to programs within the community and public agencies,
- Case Manager that will work with each participant to obtain any essential documents (i.e. valid identification, driver’s license); as well as work with the individuals to address any other issues such as record expungement,
- Once the participant has been released from custody, the case manager will make any necessary referrals for services that were not completed prior to release,
- The case manager will monitor the progress of the participant and address any issues that are hindering the participant from completing their goals,
- The case manager will have regular meeting with assigned to the project to address any issues that are hindering progress of the participants.
(2) A pre-proposal meeting will be held on January 31, 2020.
(3) All the questions must be submitted no later than February 6, 2020.

- Onshore (USA Only);
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
