[A] Budget: Looking for Proposal
[B] Scope of Service:
Vendor needs to provide interim case management entity services for planning and delivering services for individuals with intellectual disabilities and or other developmental disabilities to the government authority located in Salem, OR.
- CME must maintain enough qualified service coordinators or personal agents to deliver the required case management, assist CME management in monitoring the quality of services, assist CME management in identifying existing and insufficient service delivery resources and option.
- CME must maintain enough qualified Assessors to conduct assessments on Individuals that are eligible to receive Services in compliance with applicable OARs, CFRs, and DHS policies and procedures.
- CME must meet the following standards Assure eligibility for Developmental Disabilities Services for Individuals, Conduct Service planning consistent with the standards described in OAR Chapters 411 through 415, Monitor the Services and supports, Conduct Service entry, exit, and transfer of Individuals per rule expectations, Conduct and implement abuse or neglect investigations and ensure any subsequent Protective Services are properly completed, Initiate civil commitment services as needed, Complete information and referral services accurately and timely, Assist Individuals and their families in accessing needed Services, Coordinate Services with DHS’ Child Welfare, as needed, Coordinate Services for school-age children with a Child’s school, as needed, Enroll Individuals accurately and on a timely basis in DHS’ Express Payment and Reporting System (“eXPRS”) and other data systems, Provide choice to eligible Individuals consistent with DHS policy and procedure unless the Individual refuses enrollment, Report serious abuse or neglect incidents to DHS using the centralized abuse management system.

- Onshore (USA Only);
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location
