[B] Scope of Service:
Vendor needs to provide social services referral and case management program will take advantage of the unique experiences of City staff and affiliated program staff that will help to link at-risk residents to needed social services to the government authority located in Chelsea, MA.
- Case manager works with the resident, relevant social service agency(s), and/or the city HUB to ensure connections to services are made.
- Management and case managers of the responding agency should have proficiency (verbal and reading) in Spanish and have demonstrated experience working with individuals with mental health issues, substance use disorders, and economic hardships.
[C] Eligibility:
- Onshore (USA Organization Only);
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location
