[A] Budget: Looking for Proposal
[B] Scope of Service:
(1) Vendor needs to provide case management and recovery support services to the government authority located in Castleton-on-Hudson, NY.
- Assessment: using an evidenced based risk and needs assessment tool to enhance the current screening and assessment process to include an initial screening for trauma to assist in expediting the referrals to existing veteran administration services.
- Hire staff to conduct structured clinical interviews of veterans who are not eligible for services through the veteran’s administration, develop a comprehensive treatment plan and conduct evidence-based seeking safety sessions focusing on psychoeducation and coping skills.
- Placement: identifying appropriate services, including but not limited to substance use treatment and MAT services, tailored to each adult veteran participant and coordinating placement in those services (such as job training, education, mental health and medical care, therapeutic services).
- Monitoring: ongoing case coordination with the intent of providing continuity and ongoing individualized services to the veteran treatment court participant and comprehensive reporting to the drug treatment court.
- Recovery Support Services: enhance the integration of peer mentors to help support client reintegration and recovery by providing specific training to the peer mentors and case management staff.
- Care Coordination: connect participants to health homes based on client’s medical needs.
- Staff will maintain project logs, reports and records in appropriate files and database(s) and provide continuous feedback to participants’ progress with team members.
- Assist in collecting and maintaining all data as required.
- Provide appropriate supervision and oversight for any staff included on this project.
