Government Authority located in Boise, Idaho; USA based organization looking for expert vendor for independent living skills and case management services.
[A] Budget: Looking for Proposals
[B] Scope of Service:
(1) Vendor needs to provide independent living skills and case management services on an as needed basis to the government authority located in Boise, ID.
- Provide Independent Living Skills and Case Management services for non-foster care youth/young adults within the age range fourteen to twenty-one (14-21) as well as Independent Living Skills and Case Management services for Independent Living (IL) eligible foster care youth/young adults within the age range of seventeen to twenty-one (17-21) that are still in foster care but are transitioning out of care.
- Develop and document in the Department’s Ensuring Safety and Permanency in Idaho (ESPI) system, IL Service Plans for all non-foster care youth within thirty (30) calendar days of referral using all required domains as outlined in the Department’s Standard for Working with Older Youth as provided by the Department.
- Provide a mentoring program to assist referred youth in acquiring the behaviors, skills, values, and attitudes that will help them achieve their positive life goals.
- Provide mentoring activities that are in conjunction with the referred youth's IL Service plan.
- Workshops for Case Management, and Foster Care Youth as referred by the Department, that provide instruction related to employment applications, resumes, employment resources, personal appearance, interviewing, work ethic, time management, home safety, home rental, lease signing, budgeting, and self-care issues including mental health and medical resources, and higher education resources such as: Scholastic Aptitude Test (SAT), college applications, Free Application for Federal Student Aid (FAFSA), tutoring; i.e. Upward Bound, Talent Search, and Student Support Services (TRIO) or Student Tuition Assistance and Revenue Trust (START) programs.
- One to two-hour meetings for up to six youth or more and up to twelve meetings per contract year (usually on a monthly basis) as needed for group guidance and assist in the facilitation of the FYI Group process and goals.
- Provide transportation for the youth if necessary, to gather required documents for ETV application.
- Assist youth in identifying necessary resources i.e.: General Education Diploma (GED), secondary education facilities, vocational institutions, and other higher education institutions.
(2) All the questions must be submitted no later than October 21, 2020
(3) The contract period will be for one year.
(4) Pre-proposal conference will be held on October 14, 2020
