Vendor needs to provide case management services to the government authority located in Paterson, NJ.
- The opioid crisis is an evolving drug epidemic that impacts the health of individuals and the safety of communities,
- Provide a comprehensive case management assessment that addresses life domains such as finances, employment, health care,
- Promote an individual’s recovery to his/her fullest potential by facilitating assessment of skills, educational opportunities, job readiness skills,
- Coordinate needed interventions and services provided by multiple agencies,
- Provide training to clients on the recognition of relapse triggers and strategies to deal with avoiding a relapse,
- In collaboration with the individual, develop a viable recovery plan,
- Educate the individuals on how to appropriately navigate treatment, social service and recovery support systems,
- Be a positive role model.

Onshore (USA organization Only)
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office premises.
