[B] Scope of Service:
Vendor needs to provide case management services for residents of the acre road housing development to assist in relocation of the residents to the government authority located in Marrero, LA.
- Work to facilitate resident care by assessing resident’s needs, evaluating housing and community options, creating plans, coordinating services and moving schedules, and gauging progress, assist with the relocation process for residents including, but not limited to, resident and community engagement, creation of individual relocation plan for each resident, assistance with applications for housing and/or to financial resources, evaluation and monitoring of resident relocation process, working with nan MCKAY staff, assist residents with completing the required and necessary paperwork to apply for a housing choice/section 8 voucher as required by HUD.

- Onshore (USA Organization Only);
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
