[A] Budget: Looking for Proposals
[B] Scope of Service:
Vendor needs to provide social services and case management services to the government authority located in Middlesex, NJ.
• Only incorporated non-profit social service agencies are eligible for this funding.
• The successful bidder shall submit quarterly reports (programmatic and fiscal) to the Middlesex County Office of Human Services within ten (10) days of the end of each quarter, and provide input as requested for the Projected Units of Service upon notice of award.
• A certified copy of the Agency’s most recent audit shall be included in the bid response.
• The successful bidder shall arrange for an annual audit of agency’s fiscal transactions and fund balances, to be conducted by an independent certified public accountant firm. The costs of the said audit shall be paid by the social service agency and a certified copy shall be forwarded to the Middlesex County Office of Human Services. Said audit shall be submitted subsequent to the final payment but in no case later than July 1, 2022.
• The social service agency shall also agree to permit an authorized County representative to visit its facilities and operations for service evaluation purposes.
• The social service agency shall defend, indemnify and otherwise save harmless the County of Middlesex, its agencies, departments, bureaus, boards, officials and employees from any and all claims or actions at law, whether for personal injury, property damage or liabilities, including the costs of defense (a) which arise from acts or omissions, whether negligent or not, of the social service agency, its officials, agents, employees, servants, subcontracts, volunteers, material suppliers or others working for or on behalf of the social service agency or (b) which arise from any other failure to perform the social service agency’s obligations under this contract or any improper performance.
• Awarding of this bid is subject to the availability of monies.
• Title II of the Americans with Disabilities Act (ADA) requires that all programs, services and activities, which are contracted out by a governmental agency, shall be accessible to a person with disabilities. The Social Service Agency shall comply with the provisions of Title II of the ADA. The agency shall submit a written plan to the county which describes the method in which County funded programs, activities, or services shall be provided to a disabled individual, as defined in the Act. Said plan shall be submitted to the Middlesex County Office of Human Services prior to the execution of this contract. The agency further ensures that it shall not discriminate against disabled persons in any aspect of employment, inclusive of the application process, hiring, training, advancement and wages, benefits or employer-sponsored social activities.
• The agency and subcontractor(s) shall comply with the County’s Affirmative Action regulations and submit all necessary documentation prior to the execution of this contract by the County.
• The social services agency agrees to provide the said social services without regard to the race, color, religion, gender, age, sexual orientation, disability or national origin of the applicant, or recipient of services.
• The social service agency agrees to comply with the provisions of the Health Insurance Portability and Accountability Act of 1996 (HIPPA). Furthermore, the social service agency agrees to initiate HIPPA Business Associate Agreements (BAA) with the County regarding certain protected health information, if deemed necessary under the provisions of the overall service agreement.
• If awarded this bid, it is mandatory that the successful bidder attend a minimum of 80% of the Middlesex County Housing Continuum of Care Committee (MCHC) meetings for the contract term, and be able to demonstrate active collaboration with other members of the Middlesex County Housing Continuum of Care.
- The contractor or subcontractor agrees to inform in writing its appropriate recruitment agencies including, but not limited to, employment agencies, placement bureaus, colleges, universities, and labor unions, that it does not discriminate on the basis of age, race, creed, color, national origin, ancestry, marital status, affectional or sexual orientation, gender identity or expression, disability, nationality or sex, and that it shall discontinue the use of any recruitment agency which engages in direct or indirect discriminatory practices.
