[A] Budget: Looking for Proposal
[B] Scope of Service:
Vendor needs to provide case management, and managing programs serving the homeless population to the government authority located in Lake Forest, CA.
- Field Outreach Services shall consist of the following but are not limited to: weekly field visits to all areas of the community identified as having unsheltered persons with the goal of making initial contact or follow-up on active cases, conduct field visits and follow-ups, as requested by the city’s program coordinator or the orange county sheriff’s department homeless liaison officer, conduct vulnerability index service prioritization decision assistance tool assessments in the field, develop and maintain a “by-name list” of each unsheltered person found to be residing within the community and provide the list to city’s program coordinator for internal, confidential use only, provide a bi-weekly report of field activities to city’s program coordinator, attend bi-weekly outreach engagement meetings and other meetings upon request, provide support to the orange county sheriff’s department homeless liaison officer as needed, maintain a daily log of all outreach activities and share with city’s program coordinator upon request, conduct a night-time survey of unsheltered persons at least two times per year.
- Case Management Services shall consist of the following but are not limited to: develop and maintain a case management plan for each unsheltered homeless person, engage in proactive case management that may include, but is not limited to housing program, substance abuse programs, mental health programs, employment programs, rehoming services, and other services as appropriate, identify and track milestones in each case management plan.
[C] Eligibility:
- Onshore (USA Organization Only);
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
