Government Authority located in Michigan; USA based organization looking for expert vendor for case management services and education training voucher.
[A] Budget: Looking for Proposals
[B] Scope of Service:
Vendor needs to provide case management services and will administer the education and training voucher program to the government authority located in Michigan.
- The Contractor must provide services described herein in the following geographic area: Statewide.
- The Contractor must assure that appropriately credentialed or trained staff under its control, including Contractor employees and/or subcontractors, perform functions under this Contract.
- Provide an ETV application to a youth within 48 hours of his/her request. The application materials must also be available online on the Contractor's website. The Contractor must mail application materials to youth without access to a computer.
- Within five working days, Contractor must make a request to MDHHS – Foster Care, Guardianship, and Adoption Program Office for proof or denial of eligibility based on age and time in foster care.
- Within five days of receiving eligibility from MDHHS, assess the youth’s application to determine if youth meet requirements for award. This is done by evaluating the application packet.
- Assess youth’s needs at time of application in the areas of education, employment, housing, and health, at a minimum.
- Disburse funding, up to $5,000.00 per year per eligible youth, for school-related costs and living fees through the Contractor's centralized financial system.
- Contact every ETV recipient via phone and/or email at least twice per year to monitor each youth's educational, housing, employment, and physical and mental health needs.
- Identify and coordinate use of resources that meet the youth's needs to better access a post-secondary education.
- Provide face-to-face case management, at a mutually agreed upon location by the youth and the Contractor, to assist with developing educational and career goals, determining living arrangements, and determining financial needs.
[C] Eligibility:
- Onshore (US Organization Only);
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
