Government Authority located in Michigan; USA based organization looking for expert vendor for case management, housing assistance, educational support services.
[A] Budget: Looking for Proposals
[B] Scope of Service:
(1) Vendor needs to provide case management, housing assistance, mentorship, and pre-college education services for youth in transition from foster care or a delinquency placement to independent living that have a closed MDHHS case including, but not limited to youth homeless/housing programs, programs for disabled youth, school to work programs, employment programs, sexual health, pregnancy prevention and abstinence programs and health care services before utilization of YIT funds to the government authority located in Michigan.
- Closed Case Services are intended for Youth who are primarily self-supporting and maintaining an independent household. Closed Case Services may include Case Management, Housing Assistance, and Employment Support.
- Have a closed foster care case and were eligible for YIT funding while the case was open
- Have been in an eligible foster care or non-secure delinquency placement at age 14 or older but are no longer under court ordered MDHHS care and supervision.
- Eligible placements include licensed foster family homes, relative provider homes, group homes, emergency shelters, child care institutions for less than 25 children, independent living, and semi-independent living placements.
- Are youth who meet Closed Case Services eligibility residing with biological parents.
(2) All the questions must be submitted no later than May 7, 2021

- Onshore (US Organization Only);
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
