Government Authority located in Kensington, Maryland; USA based organization looking for expert vendor for case management services.
[A] Budget: Looking for Proposal
[B] Scope of Service:
(1) Vendor needs to provide exec case management services to a permanent supportive housing population of up to 57 single adults with chronic mental illness to the government authority located in Kensington, MD.
- Case Managers should promote self-responsibility among clients and ensure that they are held accountable for any non-compliance with program rules. Case Managers will assist the participant to assimilate into the community and help motivate participants to reach their maximum potential.
- Staffing of two full-time Case Managers to provide case management services for 57 single
adults who have mental health, physical health, and/or substance abuse disabilities with
a history of homelessness who are participants in the Supportive Housing Program.
- Case Manager must provide between one to four face-to-face in-home visits with clients each month to address risks to housing and assist with client’s compliance with Program requirements. As part of the visit, the Case Manager will walk through the client’s unit to ensure that it is well maintained.
- Case Managers will ensure on an annual basis that each client completes and signs Release
of Information forms for communication with HOC staff, HMIS, and other relevant
professionals.
- Case Manager will assist with the facilitation of annual unit inspections and re-certifications.
- Case Managers shall conduct client assessments upon contract initiation and evaluate
existing client individual service plans. Additionally, Case Managers will adjust or create new
goal plans as every 90 days.
- Case Managers will complete a budget with each client. The budget will be updated annually whatever is needed by the client. And shall accommodate the payment of rent invariably. On a monthly basis, Case Managers shall monitor client non-payment of rent and address as a budgetary and counseling issue.
- Case Mangers will assist clients with applying for and obtaining all entitlement benefits for
which they are eligible.
- The Case Managers will monitor and document the client’s compliance with: service and treatment plans, progress toward goals, medication management, program requirements (i.e., failure to pay rent) and lease requirements (i.e., community disturbance, damages to unit, etc).
- Case Management should intercede when crises or problem occurs in an effort to restore
stability to the participant.
(2) All question must be submitted no later than August 05, 2019.

Performance of the work will be Offsite. Vendor needs to carry work in their office location
