(1) Vendor needs to provide intensive and transitional case management services to the government authority located in Utica, NY.
- Increase permanency and stability for children
- Decrease utilization of out of home placements
- Utilize the least restrictive placement and service interventions when necessary
- Increase the use of natural supports, the utilization of positive and normative activities and experiences to promote prosocial behaviours, and empowerment strategies to enhance individual and family functioning Improve behavior and decrease mental health symptoms
- Improve school outcomes, as measured by improved grades, increased involvement in school and extracurricular activities or improved attendance etc.
- Reduce child and family risk factors
(2) Contract term will be one year.
