[A] Budget: Looking for Proposal
[B] Scope of Service:
(1) Vendor needs to provide case management services to intensive outreach-based case management and peer recovery support, individual service plans, and referral to comprehensive services including, housing resources, mental health counseling, substance use treatment, education and job development to the government authority located in Columbia, MD.
- Case managers shall provide services to the participants using a street outreach model which includes offering services in the community and on location to conduct follow-up services.
- capability to bill Medicaid for case management services once the grant ends.
- Case managers shall assess whether the participant is intoxicated and/or if the participant can make a clear decision
- Case managers shall reassess and update the plan every 45 days and record services provided and referred to at each participant contact.
(2) A pre-proposal teleconference will be held on August 26, 2021.
(2) All questions must be submitted no later than August 27, 2021.
[C] Eligibility:
- Onshore (USA Organization Only);
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
